1. WHO MAY PLAY
No Purchase Required. Must be 18 or over on the date of entry. Photo ID required. Contest open to legal Nevada residents except employees of Siems Eye Center, Letizia Ad Team and the immediate families of those employees. The term „immediate family members‰ includes spouses, parents, grandparents, siblings, children, and grandchildren. One winner per person per household. No purchase necessary to enter or win. A purchase will not improve your chances of winning. All federal, local and municipal laws and regulations apply. Void where prohibited. By participating, you agree to these Sponsor‚s Official Promotion Rules and to the decisions of the judges which are final and binding in all respects.

2. CONDITIONS OF WINNING
Official rules are available at Siems Eye Center, Monday through Friday 8am to 5pm. By entering the contest, contestants give permission for their name, photo, likeness and/or voice to be used for promotional purposes without compensation. Contestants also give permission for solicitation via phone, email, or mail by Siems Eye Center. Winners must sign promotional release form as condition of receiving prize. Prizes are non-transferable and cannot be redeemed for cash value.

3. CONSTRUCTION
The invalidity or unenforceability of any provision of these rules shall not affect the validity or enforceability of any other provision. In the event that any such provision is determined to be invalid or otherwise unenforceable, these rules shall be construed in accordance with their terms as if the invalid or unenforceable provision was not contained therein.

4. METHOD OF ENTRY/PRIZES OFFERED
Beginning August 28th, 2009 and lasting until September 25th, 2009, people can enter online at siemslasik.com. Limit one entry per person. If contestant has Lasik surgery performed in the month of September, contestant will receive 10 entries. All contestants must be present to win. Entry is valid only the day registration form is completed and is invalid thereafter. Entries are limited to one per day per household. Winner will be chosen September 26th at 12pm at Siems Eye Center, 10777 W. Twain Ste #150. Winners may waive their right to receive prizes. Prizes are non-assignable and non-transferable. Prizes and individual components of prize packages are subject to availability and sponsor reserves the right to substitute prizes of equal or greater value. For the name of the winners, send a separate, self-addressed stamped envelope to: Siems Eye Center Winner, 10777 W. Twain Ste #, Las Vegas, NV 89135. Siems Eye Center reserves the right to amend any of the rules at any time during the contest without notice. PRIZE(S) / ODDS: The Grand Prize of IntraLase Lasik Surgery. Odds of being chosen as an official Siems Eye Center finalist will vary depending how many people play or register for each contest. UNCLAIMED PRIZES WILL NOT BE AWARDED. Ten entrants will receive a secondary prize of $1000 off Custom IntraLase Lasik Surgery. All contestants will receive a free Lasik screening.

5. TIME PERIODS
Beginning August 28, 2009 and lasting thru September 25, 2009 registration will be open online at SiemsLasik.com. Winners will be chosen at random at 12pm at Siems Eye Center, 10777 W. Twain Ste #, Las Vegas, NV 89135.

6. Winner‚s Taxes & Financial Responsibilities
Prize winner(s) is/are solely responsible for any and all local, state and federal taxes on the grand prize.
